At Augsburg Fairview Academy, the safety and well-being of our students, staff, and community are our top priority. If you see, hear, or experience something that doesn’t feel safe, we want to know about it.
This Anonymous Safety Report Form allows anyone—students, families, staff, or community members—to share concerns confidentially. Examples of what you can report include:
- Bullying, harassment, or intimidation
- Threats to personal or school safety
- Concerns about self-harm or the well-being of others
- Weapons, drugs, or other dangerous situations
- Any behavior or situation that feels unsafe
All reports are taken seriously. Submissions go directly to school leadership and are reviewed promptly. While the form is anonymous, you may choose to include your name or contact information if you’d like follow-up support.
